Patients usually describe chest pain, not heart pain.

Use polite and professional examination language.
The patient is bad
The patient is clinically unstable / deteriorating
Use specific clinical language.
I did bloods
I requested blood tests / took bloods
Use clear verbs depending on context.
The patient denied from fever
The patient denied fever
Do not use 'from' after denied.
He is complaining from
He is complaining of
Use 'complaining of' for symptoms.
She is on metformin since 5 years
She has been on metformin for 5 years
Use present perfect with duration.
